here is the translation for
I was on leave from 13th to 30th November. That is the reason for the late reply.
To be more polite, you can say,
I apologise for the late reply, as I was on leave from 13th to 30th November.
I apologise for not being able to get back to you earlier, as I was on leave from 13th to 30th November.

no offence to anyone:
002 'sorry for late replied' is wrong.
It should be: 'Sorry for the late reply": 'reply' here is a noun; this sentence is more suitable for conversation rather than written reply;
002 and 003 'the' is necessary as 'the' refers to 'this reply' you are writing.

004 'I have holidays ....' is wrong. 'have' should be in the past tense: 'had'
005 'response' is wrong to mean 'reply'

Sorry for my late response since I was in vacation between 13th and 30th of November.

Mee, mine is business English. And yours is school simple presentation. 直頭係直譯.. Please respect to others, okay?
Officially it should be responded to the case/duty, not focus on "reply email". 意思係作出回應, 唔應該著眼於"回一封電郵"。 =.=資料來源： 小Nick
